Your Privacy, Rights & Practice Notices
CTA Behavioral Health provides important notices and policies to help clients understand their privacy rights, communication options, financial and treatment-related responsibilities, and how the practice operates. These materials are intended to support informed participation in care and provide clear information about how to contact the practice with questions or concerns.
Notice to Clients
Clinical services are provided by Candice Fuimaono, Licensed Marriage and Family Therapist, California License #140872. The California Board of Behavioral Sciences receives and responds to complaints regarding services provided within the scope of practice of marriage and family therapists. You may contact the Board online at www.bbs.ca.gov or by calling (916) 574-7830.
Notice of Privacy Practices
CTA Behavioral Health maintains a separate Notice of Privacy Practices explaining permitted uses and disclosures of protected health information, client privacy rights, CTA Behavioral Health’s responsibilities, how to request records or amendments, how to request confidential communications or restrictions, how to request an accounting of certain disclosures, and how to make a privacy complaint.
Electronic Communication & Text Messaging
Standard text messaging is used only for limited administrative communication, such as scheduling, brief logistical information, or directing you to the secure Sessions Health portal. Please do not send detailed clinical information, treatment concerns, records, or other sensitive health information by ordinary text message. Substantive clinical communication should occur through Sessions Health or another approved secure method. Text messaging is not monitored as a guaranteed real-time crisis service.
Telehealth Services & Privacy
CTA Behavioral Health provides psychotherapy by telehealth to clients who are physically located in California at the time of service. At each telehealth appointment, the clinician may confirm the client’s identity, current location, privacy, and appropriate emergency or callback information. Clients should participate from a private setting whenever possible. If the connection is interrupted or telehealth is no longer clinically appropriate, CTA Behavioral Health will use the established callback, contingency, or referral process.
Emergency & Crisis Support
CTA Behavioral Health is not an emergency or crisis-response service, and routine text messages, email, and portal messages are not monitored as guaranteed real-time crisis channels. If you are experiencing an immediate emergency or believe you or someone else may be in imminent danger, call 911. For urgent mental health crisis support, you may also call or text 988 or go to the nearest emergency department.
Child & Adolescent Services
CTA Behavioral Health uses separate consent, privacy, caregiver-involvement, and communication procedures for clients ages 6–17. For children and adolescents, the appropriate consent pathway, caregiver role, confidentiality expectations, and access to information are reviewed based on the young person’s age, circumstances, and applicable law. A parent or caregiver’s involvement or payment for services does not automatically provide unrestricted access to all clinical information.
Accessibility & Nondiscrimination
CTA Behavioral Health welcomes concerns regarding accessibility or discrimination. If you experience difficulty accessing this website, participating in services, or using a CTA Behavioral Health communication pathway, please contact the practice so the concern can be reviewed. Concerns may be raised verbally, through the secure client portal, by telephone, in writing, by approved email, or through an authorized representative. Raising a concern with CTA Behavioral Health does not limit your right to contact an appropriate external agency.
Privacy Questions & Complaints
Candice Fuimaono, LMFT, President, serves as CTA Behavioral Health’s Privacy Official and Privacy/Complaint Contact. Questions or concerns about privacy practices may be submitted by email to info@ctabehavioralhealth.com or by telephone at (951) 897-6123. CTA Behavioral Health will respond to the inquiry and arrange appropriate follow-up. Raising a privacy concern or complaint will not affect your right to receive appropriate services or to contact an outside agency when permitted by law.
Website Privacy Policy
CTA Behavioral Health’s Website Privacy Policy addresses privacy practices related to use of this public website. It is separate from the HIPAA Notice of Privacy Practices, which addresses protected health information used or maintained in connection with clinical care. Clinical intake, treatment records, and substantive clinical communications are handled through approved clinical systems rather than the public website.
Fees & Financial Policies
Information about private-pay fees, insurance, out-of-network superbills, Good Faith Estimates, cancellation and missed-appointment fees, and financial responsibility is available on CTA Behavioral Health’s Fees & Insurance page.
